TRUSTEE REQUIREMENTS: All Debtors/Debtor’s attorneys are required to submit: A) 2 years of tax returns; B) 6 months of bank statements C) proof of payoff on any liened vehicle D) proof of value; i.e. an appraisal, on any owned vehicle The above documents are to be received by the Trustee no later than 7 days prior to the 341 Meeting of Creditors. Pro Se Debtors should email documents to or mail them; otherwise, the documents are to be uploaded to the Trustee through Trustee Collaborative/Blue Stylus  (see directions below).  ONLY in the case of a debtor not having computer access, may the documents be sent by U.S. mail .  DO NOT CALL TO CONFIRM RECEIPT OF DOCUMENTS.

Federal Hwy, #203 Boca Raton, FL 33432 Attention: Susan Mark the Envelope with: TRUSTEE DOCUMENTS FOR UPCOMING 341 NOTE: The bank statements must be INCLUSIVE of the date of filing of the bankruptcy petition. In addition to the above document requirement, all debtors must produce a government-issued photo i.d. (such as a driver’s license or passport) AND an original social security card. If either of these documents is not available at the 341 Meeting of Creditors, the MEETING WILL NOT TAKE PLACE. DEBTORS NEEDING A REPLACEMENT SOCIAL SECURITY CARD SHOULD CONTACT THE TRUSTEE’S OFFICE DIRECTLY FOR FURTHER INSTRUCTIONS.

CREDITOR MEETING FOLLOW-UP: In the event documents or information are requested by the Trustee at the meeting of creditors, said information must be provided to the Trustee’s office no later than 10 days from the date of creditors meeting. If you are unable to comply with the 10-day requirement, Debtors, or their counsel, must contact the Trustee’s office within the 10-day timeframe and give an estimated date by which the documents will be received. If the documents are not received within the 10-day time frame, deadline extensions will be sought with the bankruptcy court for exemptions and/or discharge. Failure to provide the Trustee with the documents requested, or to contact the Trustee’s office within the 10-day time frame may result in an action being filed under 11 U.S.C. §727(a) 3 for failure to produce books and records. STATUTORY REQUIREMENTS: The bankruptcy code requires that certain documents be filed, including payment advices for the 60 days preceding the date of the bankruptcy filing. In addition, tax returns, as set forth above, must be produced to the Trustee no later than 7 days prior to the meeting of creditors. If you fail to file and/or produce the required documents, your meeting will likely be continued. An attorney who regularly fails to file the required documents will have the matter brought before the bankruptcy court. REQUESTS FOR CONTINUANCE: Requests for continuance will be granted only for cause. Examples of cause are: attorney vacations (other than November 15th through December 31st); debtor illness or injury; family emergencies (debtors or attorneys). Continuances will not be granted for Debtor vacations, Debtors’ inability to take time off from work or similar instances. It is the Debtors’ and their attorneys’ obligation to attend the meeting of creditors.
